New Client Forms

Welcome to Guardian Equine Veterinary Services! Thank you for trusting the health and welfare of your horse with us. We look forward to getting to know you.

Please download and complete the following forms prior to your first appointment.

If your horse needs emergency treatment when you are not physically present, we will provide care if we have a signed treatment authorization form, and you will be provided with an invoice that will be due upon receipt.

Billing

Service Agreements and Payment Options

Clients are expected to pay at the time of service or provide a credit card on file with our practice. We accept cash, checks, and credit cards, as well as payment through Care Credit.

In the event that you incur an account balance, a finance charge will be added of 1.5% per month or $5.00, whichever is greater. If you do not remit payment in full within 30 days of receiving our statement, we will be unable to continue providing care for your animals until the account is brought current.

Policies

Cancellation Policy: If you fail to appear at your appointment or to cancel without 48 hour advance notice, a $85 fee will be applied to your account. This fee must be paid prior to any further appointments are to be scheduled. Please call (757) 840-8030 or email info@guardianequinevet.com if you need to reschedule your appointment.

Emergency Services: Guardian Equine Veterinary Services understands that emergencies happen and we are available to service our established clients off-schedule as such situations arise.
